<h2>Causes of incontinence</h2>
<p>Incontinence can be caused by many factors, including an inability to recognize the need to use the restroom, forgetting where the bathroom is located, medications, stress, certain physical conditions, clothing that is difficult to remove and constipation.</p>
<p>If a person with Alzheimer’s or other dementia has recently started to lose control of his or her bladder and bowels, the first and most important step is to determine possible causes.</p>
<p>If incontinence is a new problem, consult with the doctor to rule out potential causes such as a urinary tract infection, weak pelvic muscles or medications.</p>
<p>Consider medical conditions, medication side effects and obstacles that may be causing or contributing to the problem:</p>
<h2>Medical conditions that can cause incontinence:</h2>
<ul>
<li>Urinary tract infection, constipation or prostate problem</li>
<li>Diabetes, stroke or a muscular disorder such as Parkinson’s disease</li>
<li>Physical disabilities that prevent the person from reaching the bathroom in time</li>
<li>Medications and diuretics that can cause incontinence:</li>
</ul>
<p>Sleeping pills and anxiety-reducing drugs that may relax the bladder muscles</p>
<p>Drinks such as cola, coffee and tea, which can act as diuretics; a diuretic increases urination</p>
<h2>Environment and clothing obstacles that can contribute to incontinence:</h2>
<ul>
<li>Not being able to find the bathroom</li>
<li>Obstacles on the way to the bathroom, such as furniture or clutter</li>
<li>Clothing that is difficult to remove</li>
</ul>

<h3>How to Respond</h3>
<h4>▷ Be Supportive</h4>
<p>Reassure the person to reduce feelings of embarrassment. Find ways to preserve dignity. For example say, “Anyone can have an accident” instead of saying, “You wet yourself.”</p>
<p>Be matter-of-fact; don’t scold or make the person feel guilty.</p>
<p>Respect the need for privacy as much as possible.</p>
<p>Do not withhold fluids. This can cause dehydration, which can lead to a urinary tract infection, increased incontinence and agitated behavior. You may, however, want to limit fluids before bed.</p>

<h4>▷ Communiate</h4>
<ul>
<li>Encourage the person to tell you when he or she needs to use the toilet.</li>
<li>Watch for nonverbal cues, such as restlessness, making unusual sounds or faces, pacing, sudden silence or hiding in corners. These cues may indicate the need to use the toilet.</li>
<li>Use adult words rather than baby talk to refer to using the toilet.</li>
<li>Learn the person’s trigger words or phrases for needing to use the toilet. The person may use words that have nothing to do with toileting (e.g., “I can’t find the light”), but to that person, it means going to the bathroom. Communication challenges often increase in the late stage of the disease.</li>
<li>Some people do well when they are reminded to go to the bathroom on a regular schedule, perhaps every two hours.</li>
</ul>
<h4>▷ Make it easy to find and use the toilet</h4>
<ul>
<li>Clear the path to the bathroom by moving furniture.</li>
<li>Keep the bathroom door open so the toilet is visible.</li>
<li>Make the bathroom standout. For example, place colored rugs on the bathroom floor and lid covers or post a sign of a toilet on the bathroom door.</li>
<li>Make the toilet safe and easy to use. Raise the toilet seat, install grab bars on both sides, and use nightlights to illuminate the bedroom and bath.</li>
<li>Consider a portable commode or urinal for the bedroom in case of nighttime accidents.</li>
<li>Remove plants, wastebaskets and other objects that could be mistaken for a toilet.</li>
<li>Remove throw rugs that may cause a person to trip and fall.</li>
</ul>
<h4>▷ Tips for managing incontinence</h4>
<ol>
<li>Watch for nonverbal cues such as tugging on clothing, restlessness, facial expressions, pacing, sudden silence or hiding behind furniture.These cues may indicate the need to use the toilet. Identify when accidents occur, then plan for them. If they happen every two hours, get the person to the bathroom before that time.</li>
<li>Remind the person to use the bathroom just before his or her usual time. Try setting a regular schedule for toilet use. For example, help the person to the bathroom first thing in the morning, every two hours during the day, immediately after meals and just before bedtime.</li>
<li>Choose clothing that is easy to remove and to clean.</li>
<li>Consider using incontinence products, such as waterproof mattress covers, incontinence pads on the person’s bed, padded undergarments or adult briefs.</li>
<li>Give the person plenty of time in the bathroom to empty his or her bladder and bowels.</li>
<li>Run water in the sink or give the person a drink to stimulate urination.</li>
<li>Check the toilet to see if the person has urinated and/or moved his or her bowels.</li>
</ol>

<p>External incontinence Devices are products (or appliances). These are worn on the outside of the body. They protect the skin from constant leakage of stool or urine.</p>
<p>Certain medical conditions can cause people to lose control of their bowel or bladder.</p>
<p>There are several products available. The features of these different products are listed below.</p>
<p>&nbsp;</p>
<h4><span style="color: #000080;">FECAL INCONTINENCE DEVICES</span></h4>
<p>There are many types of products for managing long-term diarrhea or fecal incontinence. These devices have a drainable pouch attached to an adhesive wafer. This wafer has a hole cut through the center that fits over the anal opening (rectum).</p>
<p>If put on properly, a fecal incontinence device may stay in place for 24 hours. It is important to remove the pouch if any stool has leaked. Liquid stool can irritate the skin.</p>
<p>Always clean the skin and apply a new pouch if any leakage has occurred.</p>
<p>The device should be applied to clean, dry skin:</p>
<p>Your health care provider may prescribe a protective skin barrier. This barrier is usually a paste. You apply the barrier to the skin before attaching the device. You can put the paste in the skin folds of the buttocks to prevent liquid stool from leaking through this area.<br />
Spread the buttocks apart, exposing the rectum, and apply the wafer and pouch. It may help to have someone help you. The device should cover the skin with no gaps or creases.<br />
You may need to trim the hair around the rectum to help the wafer stick better to the skin.<br />
An enterostomal therapy nurse or skin care nurse can provide you with a list of products that are available in your area.</p>

<h4><span style="color: #000080;">URINARY INCONTINENCE DEVICES</span></h4>
<p>Urine collection devices are mainly used by men with urinary incontinence. Women are generally treated with medicines and disposable garments.</p>
<p>The systems for men most often consist of a pouch or condom-like device.</p>
<p>This device is securely placed around the penis. This is often called a condom catheter.</p>
<p>A drainage tube is attached at the tip of the device to remove urine. The drainage tube empties into a storage bag, which can be emptied directly into the toilet.</p>
<p>Condom catheters are most effective when applied to a clean, dry penis. You may need to trim the hair around the pubic area so the device attaches better.</p>
<p>You must change the device at least every other day to protect the skin and prevent urinary tract infections. Make sure the condom device fits snugly, but not too tightly. Skin damage may occur if it is too tight.</p>

<h4><span style="color: #000080;">Female Incontinence Devices</span></h4>

<p>There are three main categories of Female Devices.</p>
<h5>External urethral devices:</h5>
<p>Products that are applied over the urethra at the opening</p>
<p>These devices are intended to work by using adhesive or mild suction to stop urine leaking from the urethra. Although they can work well for some women, they are only suitable for use with women who are keen to use them and have good use of their hands.</p>
<p>There have been a number of research studies to see how well external urethral devices work 12345[678</p>
<p>However, none of these devices are currently available on the market. Although new products become available from time to time, they have never gained widespread popularity.</p>
<h5>Internal urethral devices:</h5>
<p>Products that are placed inside the urethra</p>
<p>Internal urethral devices are small devices inserted into the urethra to temporarily block the passage of urine. They are not widely used although some women find them useful.</p>
<p>Internal urethral devices can be hazardous if used incorrectly</p>
<h5>Internal vaginal devices:</h5>
<p>Products that are inserted into the vagina</p>
<p>Internal vaginal devices are placed inside the vagina to support the bladder neck thereby reducing/preventing stress urinary incontinence.</p>
<p>Internal vaginal devices are also referred to as ‘intravaginal devices’. This term is used to describe a variety of devices which are designed to support the bladder neck to improve stress urinary incontinence (SUI).</p>
<p>Some devices are also thought to compress the urethra which also helps to reduce SUI.</p>

<h2><span style="color: #000000;">The Increasing Prevalence of Drug Problems among Seniors. The Quiet Epidemic of Senior Drug Abuse.</span></h2>
<p>The precise rate of seniors and elderly people with drug problems is difficult to assess.</p>
<p>One reason is that “many of the signs and symptoms of misuse and abuse mirror common signs of aging in general” as Belinda Basca notes in an article for the periodical Prevention Tactics.</p>
<h3><span style="color: #000000;">Recently released statistics present clear hints at a growing epidemic of drug abuse..The Quiet Epidemic.</span></h3>
<p>According to the Prevention Tactics report, “prescription drug abuse is present in 12 to 15% of elderly individuals who seek medical attention.</p>
<p>What’s more, a document from the Johns Hopkins Medical School notes that the number of Americans over age 50 abusing prescription drugs is projected to rise to 2.7 million in 2020 – a 190% increase from the 2001 figure of 910,000.</p>
<h3>Drinking Problems and Seniors:</h3>
<p><img loading="lazy" decoding="async" class="aligncenter wp-image-8320 size-full" title="The Quiet Epidemic" src="https://www.ortohispania.com/wp-content/uploads/2016/08/The-Quiet-epidemic.jpg" alt="The Quiet Epidemic" width="200" height="213" data-id="4556" />Alcohol abuse and alcoholism are other major issues among seniors.</p>
<p>A study published in the Journal of General Internal Medicine found that “more than a third of drinkers 60 years old and older consume amounts of alcohol that are excessive or that are potentially harmful in combination with certain disease they have or medications they are taking.” Alcohol abuse and addiction can be just as dangerous and destructive as drug addiction, so don’t turn a blind eye to an older loved one’s alcohol problem.</p>
<h3>Misuse vs. Abuse. The Quiet Epidemic:</h3>
<p>It’s worth making a distinction between drug misuse, which is common among seniors, and drug abuse which is less prevalent among the older population.</p>
<p>The majority of seniors who become dependent on prescription drugs are being treated for legitimate medical issues such as pain, anxiety, depression or insomnia. They may increase their dose against medical advice in order to seek greater relief from their condition. This is drug misuse.</p>
<p>Drug abuse is less common among seniors, but is a big issue none-the-less. As one article puts it, abuse involves the “repetitive and willful habit of taking drugs for the purpose of pleasure, ecstasy and euphoria but does not include the repeated use of drugs for therapeutic purposes.”</p>
<p>This distinction is important. Seniors who are misusing medications for therapeutic purposes may be doing so because their current treatment plan simply isn’t effective in addressing their symptoms. Misuse or overuse can be controlled if physicians reevaluate treatment options so that patients don’t rely on more medication than prescribed in order to get full relief.</p>
<h3><span style="color: #000000;">Drug abuse on the other hand can be more difficult to get under control, and may require drug treatment.</span></h3>
<h3>Dependence vs. Addiction:</h3>
<p>Just as there is a distinction between misuse and abuse, there is a distinction between dependence and addiction. Many medications are physically addictive, such as opioid painkillers like Vicodin or Percocet. Sedative, anti-anxiety, and insomnia drugs like Xanax and Valium can also cause physical dependence when taken daily, even at prescribed doses. Even without any misuse or abuse, a patient who is physically dependent will experience uncomfortable drug withdrawal symptoms if they abruptly stop taking the medication. If a patient stops medication that they are dependent on, their doctor will often prescribe gradually decreasing doses (a “taper”) to reduce discomfort.</p>
<p>Addiction is more often the result of drug abuse. Addicts, seniors and otherwise, are usually not only physically dependent on the drug or drugs they are taking, but also take them clearly compulsive and harmful way. As the National Institute on Drug Abuse “compulsive drug use despite harmful consequences—is characterized by an inability to stop using a drug; and failure to meet work, social, or family obligations.”</p>
<h3>Drugs of Misuse Among Seniors:</h3>
<p><img loading="lazy" decoding="async" class="aligncenter wp-image-8318 size-full" title="The Quiet Epidemic" src="https://www.ortohispania.com/wp-content/uploads/2016/08/drug-abuse-elderly.jpg" alt="The Quiet Epidemic" width="183" height="275" data-id="4553" />Prescription drug and misuse and abuse and alcoholism are much more of an issue for seniors than abuse of hard street drugs like heroin or cocaine. Being aware of the primary classes of medicines that lead to dependence or addiction can be helpful:</p>
<p><strong>Opioids:</strong> Opioids are used to treat pain. Common opioids include oxycodone (the active ingredient in Percocet and Oxycontin), hydrocodone (the active ingredient in Vicodin and Norco) and numerous other related medicines such as morphine, codeine, hydromorphone and fentanyl. Opioids are a vital tool for pain management, but carry clear risks of dependence and addiction. The number of fatal opioid over doses has risen dramatically over the last decade.</p>
<p><strong>Benzodiazepines:</strong> Benzodiazepines are a class of medicines primarily used to treat anxiety, panic disorders, insomnia – and can also be used to treat bipolar disorder and even epilepsy. Common benzodiazepines include diazepam (Valium), alprazolam (Xanax), lorazepam (Ativan) and clonazepam (Klonopin). This class of medications can be highly habit forming, dangerous when mixed with other sedatives, and abrupt withdrawal can lead to seizures and other major medical issues.</p>
<p><strong>Alcohol:</strong> Alcohol is undoubtedly a drug, but because it’s considered socially acceptable, seniors with alcohol problems can fly under the radar. Alcohol abuse can lead to dementia, liver failure, and other serious and potentially lethal health problems and should not be ignored. What’s more, alcohol affects seniors more strongly than younger people, so seniors should be particularly prudent with alcohol.</p>
<p><strong>Stimulants:</strong> Stimulant such as Ritalin or Adderall are often prescribed to younger people for attention deficit hyperactivity disorder (ADHD), and sometimes prescribed to older adults for disorders such as narcolepsy and obesity. Stimulants can be habit forming and stimulant abuse can be highly injurious to health, particularly among seniors.</p>
<h3>Signs of Drug Problems:</h3>
<p>Here are some signs that can indicate that your older loved one may have a substance abuse problem:</p>
<ul>
<li>Appearing over sedated, disoriented or impaired</li>
<li>Poor balance or unsteady gait</li>
<li>Requesting early refills</li>
<li>Reporting that their medications have been lost or stolen (particularly if this occurs more than once)</li>
<li>Poor hygiene or disheveled appearance</li>
<li>Appetite changes</li>
<li>Mood swings or major personality changes</li>
<li>Increased isolation</li>
<li>Demanding narcotic drugs at visit to the doctor</li>
<li>Apparent doctor shopping</li>
<li>It’s important to note that any of these signs and symptoms could be due to reasons besides drug problems, but they are definitely cause to investigate further.</li>
</ul>
<h3>Help for an Older Loved One with a Drug Problem:</h3>
<p>If you believe an elderly loved one may have a drug problem you should intervene.</p>
<p>One option is alerting your loved one’s physician about your concerns.</p>
<p>The right type of treatment will vary as well as the individual and the circumstances.</p>
<p>A hopeful trend is the emergence of drug treatment programs especially for seniors, which was born out of the recognition that there are differences in the drug treatment strategies that are effective for seniors as compared to younger people.</p>

<p>This Saturday, March 14, International Urinary Incontinence Day is celebrated.</p>
<p>A syndrome characterized by a loss of bladder control that, whether as a consequence or as a symptom of different diseases,</p>
<h3>Around seven million people in our country suffer incontinence of which more than four million are women &#8211; around 25% of women over 35 and up to 50% of those over 65.</h3>
<p>Urinary incontinence has effects not only physically, but also psychologically and socially, having a negative impact on the quality of life of those affected.</p>
<p>In fact, studies have shown that women with urinary incontinence not only have a higher risk of suffering from depression, but also present greater difficulties in carrying out different daily activities &#8211; including, among others, the practice of exercise, of having sex or, simply, walking &#8211; which entails a feeling of insecurity and a serious loss of self-esteem.</p>
<h4>The result is that 86% of the affected population fears social rejection and, consequently, deliberately limits their public activities.</h4>
<p>Seven million Spaniards suffer urinary incontinence 50% of cases registered in Spain do not receive adequate treatment</p>
<h4>Urinary incontinence has become a true silent epidemic that affects at least 7 million Spaniards.</h4>
<h4>It is estimated that, at advanced ages, it is suffered by up to 75 percent of the population.</h4>
<p>In addition, it is one of the pathologies that, according to WHO, most affect the quality of life of patients, surpassing even diseases such as diabetes.</p>
<p>That is why, on the occasion of March 14, International Day of Urinary Incontinence, the Spanish Association of Urology (AEU), the Spanish Society of Geriatrics and Gerontology (SEGG), the Spanish Society of General and Family Physicians (SEMG ), and the Spanish Society of Family and Community Pharmacy (SEFAC), in collaboration with Ontex Healthcare, have presented the Decalogue of Urinary Incontinence.</p>
<p>It includes statements such as that 50 percent of cases of urinary incontinence do not receive adequate treatment; This is a pathology that affects 1 in 20 people, and that it is a reversible situation in most cases.</p>
<p>Likewise, it affects the negative effects of said disease at all levels in patients who suffer from it.</p>
<p>As Manuel Esteban, president of the Spanish Association of Urology (AEU) recalls &#8230;<br />

<p>Long-handled shoe horns can help with putting on and taking off shoes. They can also be used to help push socks off the lower leg.</p>
<p>As many have a hook on the alternative end, they can also be used for reaching items of clothing, such as pants and trousers.</p>
<h4>Boot jack</h4>
<p>This is a traditional way of getting boots off and can be very useful if bending is difficult. The heel of the boot is held in the jack whilst the foot is pulled up and out.</p>
<h4>Zip aids or zip puller</h4>
<p>A zip aid is usually a piece of cord or fine chain with a hook on one end and a tab at the other to assist with doing up zips. Large rings or tape can also be attached to a zip tab to help make gripping easier.</p>

<p>The flexible version is cone shaped and made of plastic, fabric-covered plastic or just fabric. It has two tapes/ribbons at the top.</p>
<p>It is sometimes called a gutter sock aid.</p>
<p>With the aid held on the knee, the sock is gathered onto the aid which keep the sock open along its whole length. The aid is then lowered onto the floor, whilst holding on to the tapes.</p>
<p>The toes of the foot are placed into the open end of the sock and the ribbons pulled to pull the sock up the lower leg.The<br />
fabric, or fabric-covered type, are recommended to anyone who has delicate skin.</p>
<p>A double version of this aid can be used for tights.</p>
<p>These are rather tricky to manipulate and require a lot of practice especially if there is a high lycra content in the tights.</p>
<p>People with poor grip or shoulder function may find pulling the tights up over the hip difficult.</p>
<p>Pulling up with hands crossed over the body reduces the amount of shoulder movement required.</p>
<h4><span style="color: #000000;">Rigid stocking/sock aid</span></h4>
<p>The rigid version consists of a plastic semi-circle which pivots on the end of a long handle.</p>
<p>As with the flexible sock aid, the sock is fed onto the aid which keeps it open.</p>
<p>The toes of the foot are then placed into the open end of the sock and the aid used to pull the sock up the lower leg.</p>
